Registration check in for the Upstate NY Number Theory Conference starts Friday, April 10th at 3:00 p.m. in room 532 Malott Hall. You will receive a packet that will include various pieces of useful information.
For those requesting funding and to expedite the process of your reimbursements it would be helpful to have copies of your receipts with you when you register on Friday (department administrative offices are closed Saturday and Sunday). Below are links to the required forms to be submitted with your receipts.

Please remember to sign and date all forms. The Form W-9 and W-8BEN can be submitted prior to your arrival at the conference by fax to 607-255-7149, Attn: Upstate Numbers Conference. Please do not email these forms.
